Maximum Tune 3DX+ is mostly on the west coast of the US as far as I know.
California has a ton of locations (Golfland / Round 1). I believe Texas has a location (Arcade UFO), Arizona has a location either at the Golfland in Mesa or some other establishment and Hawaii has a bunch of locations. Other than that most locations in the USA only have Max Tune 2 or 3. Max Tune 4 has not been released anywhere outside of Asia / Australia / New Zealand areas, so no locations for USA / Canada.

Sorry about not formatting this right, but here are a few changes and clarifications on the SoCal section, since I live there:

EDIT (08/24/14): Putting notes behind a spoiler, as I have formatted a new list seven posts down.

Round 1 - Please modify name to "Round 1 - City of Industry". "Cost for a game" to remove "1.50" at end, and modify "Card Renewal" to read "6 swipes ($3.00)". Japan Arcade is gone, so please erase that arcade from the listings. Arcade World - None of the 3 (not 4) cabinets have cards. No idea on what the status is on cabinets, since they only have 3 now. I only played #1 today (06/23/14), and it was a little dark. UPDATE (08/04/14): Looks like the manager took the "DO NOT SIT IN GAME!" from his Burbank location below, downgraded it to a Max Tune 2, and put it in the #1 spot here. That means they have only 2 out of 3 working cabinets now. All Amusement (Burbank) - Max Tune 2 machines gone, none of the Max Tune 3 cabinets have cards. Just went today (06/23/14) and #1 cabinet broken (missing steering wheel, cabinet draped in CAUTION tape and sign stating "DO NOT SIT IN GAME!") UPDATE (07/11/14): #2 - have to hold shifter for 2nd and 4th gear. UPDATE (08/13/14): Just played today, and saw #3 freeze during a battle with #4, which made #4 reset after a couple of minutes. UPDATE (08/16/14): #2 - cannot use 2nd, 4th, or 6th gear (goes to Neutral). Worker notified, and she taped up the coin mechs to have manager check it out next week. UPDATE (08/22/14): #2 still cannot shift to 2nd, 4th, or 6th, #4 not linked to other three cabinets. UPDATE (08/29/14): #1 froze on me while playing, same issues with #2 and #4 (would sometimes show linked but then unlink). Talked to manager about #2's shifter, and he said that kids play around with the shifter and it costs $800 for a new one. Mt. Vernon Swapmeet - Please modify name to "Pro Swap Meet". The address and phone number is 632 S Mount Vernon Ave, San Bernardino, CA 92410-2747, (909) 884-9700‎. According to Haxor, neither card reader is on. UPDATE (08/23/14) - Both card readers broken. Gameworks - Address and phone number is 4541 Mills Cir, Ontario, CA 91764, (909) 987-4263. Also, Max Tune 2 is no longer "at times" cannot use cards. The card readers are broken and the cabinets state this. When I went, both Max Tune 3 cabinets do not have cards. Please keep the information about asking the tech for cards. UPDATE (08/23/14): WMMT 2 - left side has no cards (but can play with cards), right side card reader broken. WMMT 3 - left side card reader broken, right side has cards. Camelot Golfland - Games are two sets of two games. #1 card reader broken, #2 has no cards, #3 and #4 out of order. Denjin Arcade - According to Yelp (yelp.com/biz/denjin-arcade-simi-valley), this arcade is gone, so please erase that arcade from the listings. Mulligan Family Fun Center no longer has WMMT, so please erase that arcade from the listings. Moreno Valley Mall location should be a Round 1 as well, so please modify name to "Round 1 - Moreno Valley Mall". According to Haxor, "4 WMMT 3dx+ JAPANESE versions. 50 cents, and well maintained". UPDATE (08/23/14): None of the four cabinets have cards. All Amusement Fun Center in "The Plant" is gone, so please erase that arcade from the listings. Family Fun Arcade is gone, so please erase that arcade from the listings. Family Arcade - Address and phone number is 876 N Vermont Ave, Los Angeles, CA 90029, (323) 660-8180. They are open from 8:30am to 1:30am. They have 4 Max Tune 3DX PLUS cabinets (all linked) and are 50 cents and 2 Max Tune 3 cabinets, which as of my last visit, both card readers are broken and the cabinets state this and are 25 cents. Make a note saying the game takes quarters and to get quarters (not tokens) from the change machines. UPDATE (07/20/14): They have removed the Max Tune 3 cabinets. UPDATE (08/04/14): New cards have gone up from $2 to $3. Net Fragz - Location is a computer LAN area. No arcade games are present. Please erase this from the listings. Malibu Speed Zone - Please change name to SpeedZone Los Angeles. Hours vary, but check speedzone.com/site/LA/directions.html for current hours. The game is now $1 per game, and they only have 4 Max Tune 3 machines, not 8. Also, none of the cabinets have cards. Be advised: Cabinet #3 likes to freeze and reset, so avoid playing. UPDATE (08/14/14): Very rarely, #4's card reader will eat your card, then yield a "Card Jam" error on the game when the machine realizes this. Ignore the attendant if they say not to put cards in there. James Games - Please add phone number (909) 946-4438. Change Fri - Sat hours to read 11:30 am - 2:00 am. Change cabinets to read: (4) WMMT 3, (2) WMMT 3 I went to James Games today (08/14/14). Even though it took me a while to find it (as it was in the back of the building), I was amazed to see the arcade still alive. The four Max Tune 3 cabinets are still there, but #4's card reader isn't working, and the other three cabinets have no cards. There is also a separate set of two Max Tune 3 cabinets, but their card readers aren't working either. I spoke with the owner, Mr. Rick Bakva, for quite some time. We talked about a lot of topics, and he was amazed how much I knew about arcades and locations. (I'm not going to bore you guys with the stories.) He told me that he ran out of cards "about four to five days ago", but he ordered more cards about three weeks ago. He should be getting cards by my next visit. When he does, I will cross out the information above. Nickel Nickel - Hours are listed on their webpage - nickelnickelhb.com/?page_id=5 . Cost of game is now only 6 nickels ($0.30), but none of the 3 cabinets (they have 4 cabinets, but the first one is off and broken) have cards. There is a $2 entry fee, unless you show them a "Like" on their Facebook page, then it's free to get in. The info about the Initial D version 4 machines is still accurate, except they do not have cards either. UPDATE (07/20/14): They no longer offer the free admission with the Facebook "Like". The best offer is now to "check-in" on Facebook (user name nickelnickelhb) for $1 admission. UPDATE (09/08/14): #1 cabinet semi-fixed (resets once in a while), but has cards for 19 nickels ($0.95), while supplies last! Arcade (No name) - Checking Google Maps at the address yields no arcade on any corner. I assume the arcade is gone, so please erase that arcade from the listings. Tilt West Covina - #1 has no cards, #2 cabinet out of order. Tilt San Diego - The address places it in the Westfield Mall. Going to their website yields no match for Tilt. I assume the arcade is gone, so please erase that arcade from the listings. NEW LOCATION Round 1 - Lakewood Center Mall Address: 401 Lakewood Center Mall, Lakewood, CA 90712, (562) 408-2937 Hours: 10am - 2am daily. 18+ after 10pm, 21+ after midnight Cost for a game: 1 swipe ($0.50) Card Renewal: 3 swipes ($1.50) Notes: These are Japanese cabinets. Notes: The cabinets are located on a U.S. Army installation, you need to go through varying levels of security to get onto the base. The card reader is currently working but is locked with the key lost so until they get a locksmith in to break it there is no way to replace cards. Also the cabinets are currently not linked. I am also semi-updating the Nor Cal section with updated information. I have already found the addresses and phone numbers of most of the locations, and have found out that Serra Bowl has closed down according to this Yelp post, and Metreon Tilt has also closed down according to this Yelp post.

According to Dantevn6 on the post below, Dave & Buster's and San Jose State University (SJSU) Student Union no longer have the Maximum Tune game, so please remove them from the list.

These are the ones I am going to try to check out some time in September.

UPDATE (09/15/14) - Updated information!

The following arcades no longer have WMMT: Classic Bowling Center, San Francisco State University.

Century City 20 Daly City requires a movie ticket to enter the theater, so added that information. Unsure if they have WMMT.

Golden Tee Golfland - Castro Valley has 4 WMMT 3DX PLUS machines, not 2.

Timeout Arcade is now a Tilt, and has gotten rid of the WMMT 2 machines, but has 3 of the WMMT 3 machines. Also, #1 cabinet has cards.

Milpitas Golfland Entertainment Center - Updated from 2 WMMT 2 machines to 4 WMMT 3DX PLUS machines, but #1 and #2 cabinets weren't working (dark screen, but marquee lights on).

I can tell you now that the San Jose State University and Dave and Busters(Milpitas) no longer have maximum tune 3 and maximum tune 1, respectively. SJSU is remodeling and they dont have an arcade anymore.
